Water is a simple molecule. Its chemical formula is H2O, which means it’s made of two hydrogen atoms bonded to a single oxygen atom. But don’t let its simple structure fool you. Water is a fascinating and crucial substance for life as we know it.

Constituting about 60% of the human body, water serves as a medium for biochemical reactions, aids in nutrient transport, helps regulate body temperature, and acts as a lubricant for joints and tissues. Maintaining proper hydration is, therefore, extremely important for overall health and optimal bodily functions, including proper thyroid function.

Individuals with thyroid disorders such as hypothyroidism may be especially susceptible to the effects of dehydration. Dehydration can also alter thyroid function tests, making it appear like you have a thyroid issue when you might not. This is because dehydration can cause changes in blood volume and protein concentration, which can affect the way thyroid hormone levels are measured. Hydration, on the other hand, can play a supportive role in managing symptoms associated with thyroid disorders, such as fatigue, constipation, and dry skin.

While we all may have gotten used to hearing of the importance of drinking water, determining how much is ‘enough’ can be a question mark for many. In this article, we will address this concern, explain how important hydration is for thyroid function and overall health, and look at some practical tips for staying properly hydrated.

Why is hydration important for thyroid function and overall health?

Water is essential for almost every bodily function, and the thyroid gland is no exception. Here’s how proper hydration supports your thyroid:

1. Nutrient and Hormone Transport

Water is essential for transporting nutrients, including iodine, to the thyroid gland. Iodine is an essential nutrient needed for thyroid hormone production. Adequate hydration facilitates this by ensuring the proper delivery of iodine to thyroid cells. 

Hydration also aids in the transport of thyroid hormones. Thyroid hormones travel through your bloodstream attached to proteins that facilitate the transport of the hormones in blood, a water-based medium. When a person is dehydrated, their blood thickens, making it harder for these proteins to move freely. This can potentially lead to a decrease in the amount of thyroid hormone available to your cells.

2. Digestion and Absorption

Proper digestion and absorption rely heavily on water. Dehydration can slow down digestion, hindering the absorption of nutrients that support thyroid health, like iodine and selenium. Dehydration can also exacerbate gastrointestinal issues associated with thyroid dysfunction, such as constipation.

3. Body Temperature Regulation

When the body is dehydrated, it struggles to regulate its temperature effectively, leading to an increased sensation of heat. This is because dehydration reduces the body’s ability to sweat and cool itself down, resulting in a feeling of being overheated. In relation to the thyroid, studies have shown that a high body temperature can lead to a decrease in circulating levels of T3, the active form of thyroid hormone.

How much water should I drink daily?

There really isn’t one universally applicable answer to this question. Think of water needs as clothing sizes – they vary from person to person. The amount of water you need can vary depending on factors such as:

  • Body weight – People with larger bodies generally need more water than those with smaller bodies.
  • Activity level – If you exercise regularly or engage in physical activity that makes you sweat, you will need to replace the lost fluids.
  • Climate – Hot and humid environments can cause you to sweat more, requiring you to drink more water.
  • Diet – Certain foods, including some fruits and vegetables,  contain water and contribute to overall fluid intake. Salty foods can increase fluid loss through urination. Certain beverages, such as coffee and alcohol, can also promote fluid loss through urination. 
  • Overall health – Certain medical conditions (such as diabetes or kidney disease) or medications (such as diuretics and laxatives) can influence how much water you need.

However, there are some common guidelines people can follow. One common recommendation is to aim for about 8 glasses of 8 ounces of water per day, which is roughly 2 liters or half a gallon. This is also known as the 8×8 rule

The U.S. National Academy of Medicine (formerly known as the Institute of Medicine) also suggests that men should aim for about 3.7 liters of total water intake per day, while women aim for about 2.7 liters of total water intake per day. This total water intake includes water obtained from beverages and food. 

While these recommendations may not be perfect for everyone, they can serve as a good starting point for many healthy adults.  

What are some strategies to stay consistently well-hydrated?

Now that we understand the importance of hydration for thyroid health and that individual water intake needs vary, here are some practical tips for staying hydrated.

  • Listen to your body: Pay attention to your body’s thirst cues and don’t wait until you feel extremely thirsty to drink water.
  • Carry a reusable water bottle: This makes it easy to sip water throughout the day.
  • Hydrate before, during, and after exercise: Drink water before, during, and after exercise to replenish fluids lost through sweating.
  • Include hydrating foods in your diet: Fruits and vegetables like oranges, watermelon, cucumber, and celery have a high water content and can contribute to your daily water intake.
  • Limit dehydrating beverages: Water is the best way to stay hydrated. Caffeinated drinks and alcohol can have a diuretic effect, meaning they increase urine production and can lead to dehydration.
  • Aim to maintain electrolyte balance: Electrolytes, such as sodium, potassium, and magnesium, play a role in hydration and thyroid function. Strive to maintain an electrolyte balance by appropriately consuming electrolyte-rich foods, especially during periods of increased sweating or physical activity.
  • Monitor your urine: Check the color of your urine to gauge hydration levels. Pale yellow urine often indicates proper hydration, while dark yellow urine may be a sign of dehydration.

Millions of people around the globe are affected by various thyroid disorders, and while genetics play a part, what you eat matters too. If you’ve been diagnosed with a thyroid disorder and are on (or considering switching to) a plant-based diet, you may have at some point wondered if this dietary choice can affect your thyroid and if there are specific foods you should avoid.

Addressing these concerns can actually be a little more complicated than expected. This is mainly because there are many different types of plant-based diets. Generally, vegetarians avoid meat, poultry, and fish, but may still consume dairy products (such as milk, cheese, and yogurt) and eggs. However, there are several variations of vegetarianism, such as lacto-vegetarian, ovo-vegetarian, lacto-ovo vegetarian, pescatarian, pollotarian, and flexitarian diets. Vegans, on the other hand, exclude all animal products from their diet, including meat, poultry, fish, dairy, eggs, and honey.

Both dietary lifestyles are usually adopted by people who are drawn to the range of health benefits these diets offer. Did you know that vegetarians and vegans tend to have lower cholesterol levels on average compared to meat-eaters? That’s a win for heart health, for sure! However, when it comes to the thyroid in particular, there are some unique considerations for those who follow a plant-based diet. 

By answering the following questions, the rest of this  article explores the link between plant-based diets and thyroid health, highlights some key nutrients vegans and vegetarians should consider, and offers tips for optimal thyroid function.

Can a plant-based diet affect my thyroid?

The short answer is yes, but it depends on the specifics of your diet and your actual nutritional needs. Now, here’s a more specific question you may have seen somewhere before: Do vegans and vegetarians have a higher risk of developing thyroid problems? Well, the answer to this isn’t so straightforward. By the end of this article, you will understand why.

Plant-based diets are by no means a “bad” thing. In fact, they have commonly been associated with effective weight management. This is mainly because they are usually lower in calories and fat. So, if you’re overweight due to your thyroid condition, a well-planned plant-based diet may be helpful in your weight management journey, alongside other lifestyle changes.

This raises the question of what a well-planned plant-based is. Simply put, it’s a plant-based diet that has been optimized to provide sufficient amounts of all the essential nutrients. This is very important because it addresses the main concern with plant-based diets within the context of thyroid health—potential nutrient deficiencies. Some essential nutrients for the thyroid, such as iodine, are not as abundant in plant food sources as they are in animal sources. 

In the next section, we will look at how you can optimize your plant-based diet for your thyroid.

How can I support my thyroid with a plant-based diet?

Over the past couple of years, research studies on the link between vegan/vegetarian diets and thyroid function have presented some interesting results. While some studies suggest a possible protective effect of vegan diets against hypothyroidism, others also suggest a likely increased risk of iodine deficiency. Iodine deficiency is, however, a major cause of hypothyroidism. 

One way to explain the seemingly contradictory or inconsistent findings is to acknowledge that the effect of a plant-based diet on thyroid function depends on several factors. In addition, some researchers have noted that goitrogenic compounds in some plant food sources may also be a cause for concern. The limitations of these studies must also be considered when analyzing and comparing their findings.

A study examining the dietary patterns of over 65,000 participants in North America in relation to the risk of hypothyroidism was published in 2011 in the Nutrients journal. The findings of this study showed that people who followed a vegan diet tended to have a slightly lower (but statistically insignificant) risk of developing hypothyroidism compared to those who ate meat. Interestingly, a lacto-ovo vegetarian diet (including dairy and eggs but no meat) showed a slight increase in hypothyroidism risk. As pointed out by the authors of the study, the reason for this result is unclear. It is nonetheless a surprising finding because if a lacto-ovo vegetarian diet can cause thyroid problems, wouldn’t vegans be at just as high a risk? Also, when you consider that lacto-ovo vegetarians are less likely to be iodine-deficient than vegans because dairy and eggs contain iodine, the findings of this study become even more interesting. It is, therefore, necessary to highlight some of the limitations of this study, such as relying on self-reported data and not measuring iodine intake directly. 

Published in the same journal in 2023 is another study that investigated iodine intake in vegans compared to omnivores in Poland. The researchers found that vegans had lower iodine intake than omnivores (90% below recommended daily intake). This was mainly because plant-based alternatives often consumed by vegans lacked iodine fortification.

To address the valid concern of nutritional deficiencies associated with plant-based diets, here are some tips to plan a plant-based diet focusing on specific nutrients to support your thyroid health as a vegan or vegetarian:

  • Iodine: Iodine is essential for thyroid hormone production. While some plant-based foods like seaweed contain iodine, it is generally less abundant in plant-based diets compared to animal-based diets including dairy and seafood. Iodized salt (in moderation for sodium intake) is a readily available source, and using it consistently is important for vegans. Vegetarians can include dairy products and eggs in their diet.
  • Selenium: Selenium is another mineral involved in thyroid hormone metabolism. It’s essential for the conversion of T4 to the more active T3 hormone. Its deficiency has also been linked to autoimmune thyroid disease. The good news for vegans and vegetarians is that with just a few Brazil nuts a day, you can easily meet the recommended daily intake of selenium. But be careful not to eat too many Brazil nuts, as this can lead to a condition called selenosis. Other sources of selenium include sunflower seeds, whole-wheat bread, and tofu. 
  • Iron: Iron is a component of enzymes involved in thyroid hormone production. Iron deficiency has been reported to worsen hypothyroidism symptoms. While leafy greens are a good source of iron (specifically non-heme iron) in plant-based diets, the iron absorption by the body is lower compared to heme iron found in animal products. Including iron-fortified foods into your diet and pairing plant-based iron-rich foods with vitamin C-rich foods (like citrus fruits, bell peppers, and broccoli) can enhance iron absorption.
  • Vitamin B12: Vitamin B12 is important for thyroid health and overall nerve and blood cell function. A deficiency of this vitamin can lead to fatigue, a common symptom of hypothyroidism. Vitamin B12 is only found naturally in animal products. For this reason, supplementing with B12 or consuming fortified foods (such as plant-based milk, breakfast cereals, and nutritional yeast) is recommended for individuals following plant-based diets.
  • Zinc: Zinc also plays a role in thyroid hormone metabolism. According to a case study published in the International Journal of Trichology in 2013, zinc deficiency can contribute to the development of hypothyroidism. This case report discussed a patient (a 28-year-old woman) who experienced both hypothyroidism and zinc deficiency at the same time, resulting in significant hair loss throughout the scalp. Some plant sources of zinc include pumpkin seeds, cashews, chickpeas, and whole grains. However, zinc from these sources is generally less bioavailable (less readily absorbed by the body) compared to non-vegetarian diets. This is because plant sources of zinc typically contain a considerable amount of phytic acid, which hinders the bioavailability (absorption) of zinc. The good news is that some meal preparation methods such as cooking, fermentation, and soaking in water overnight can help reduce the levels of phytic acid before consumption.

It’s worth noting that while a well-planned vegan or vegetarian diet can provide all the necessary nutrients for thyroid health, it’s also important to make sure your diet is varied and balanced. Rather than having a monotonous diet, try including a wide range of fruits, vegetables, whole grains, legumes, nuts, and seeds to meet nutritional needs.

Aside from the essential nutrients, here are some additional considerations for optimizing your plant-based diet to your thyroid health:

  • Consuming cruciferous and soy-based foods

If you have a hypoactive thyroid, chances are you’ve heard, at least once, that it’s best to avoid cruciferous vegetables and soy foods in order to protect your thyroid. This isn’t quite accurate. For most people with thyroid conditions, these foods are safe to eat, but with some important considerations.

Cruciferous vegetables like broccoli, kale, cabbage, and Brussels sprouts contain compounds called goitrogens, which can interfere with thyroid function (typically resulting in a goiter) when consumed in large quantities. However, this is usually only a concern for people with existing iodine deficiency. Cooking these vegetables, rather than consuming them raw, can deactivate goitrogens, reducing their potential impact on thyroid health. 

Soy products contain compounds called isoflavones, which can be goitrogenic. In large amounts, they may interfere with thyroid hormone production and absorption. However, moderate consumption of soy foods is generally considered safe for your thyroid. You can also try fermented soy products like tempeh and miso because fermentation helps break down some of the goitrogenic compounds in soy.

  • Consulting a healthcare professional

If you are on a vegan or vegetarian diet, you should consider consulting a healthcare professional, particularly a registered dietitian or nutritionist, to make sure that your nutritional needs are being met. They can perform tests to assess any nutritional deficiencies you may have. 

Remember, effective communication with your endocrinologist or thyroidologist is also important. Don’t hesitate to make your concerns known to them. In addition to monitoring your thyroid function, they can also offer personalized guidance tailored to your specific health concerns.

Impact of Thyroid Disease on Work Performance

Thyroid disorders, which affect millions of people worldwide, encompass a range of conditions such as hypothyroidism, hyperthyroidism, thyroid nodules, and thyroid cancer. According to the American Thyroid Association, an estimated 20 million Americans have some form of thyroid disease, with up to 60% of those affected being unaware of their condition, while women are up to eight times more likely to develop thyroid conditions than men. Moreover, thyroid disorders such as hypothyroidism and hyperthyroidism, especially when untreated, can disrupt some aspects of daily life, including work performance and productivity. 

Let’s face it—in today’s rapidly evolving world, the workplace is where many of us spend a significant portion of our time. Yet, despite the prevalence of thyroid disorders, thyroid health remains a topic often overlooked in workplace wellness discussions. And yes, it is true that every job is different, and the demands placed on individuals vary widely. From the fast-paced deadlines of the corporate world to the physically demanding tasks of labor-intensive jobs, the nature of work can significantly influence how thyroid health is managed on a day-to-day basis. Other factors such as the severity of thyroid symptoms, and access to healthcare resources all play a role in determining how thyroid health may influence work performance and productivity. That’s all true.

However, there are common concerns and challenges that many individuals with thyroid disorders face in the workplace. And while there’s no one-size-fits-all solution for managing your thyroid health at work, there are definitely strategies that can make a big difference.  So, whether you’re a programmer glued to your screen or a construction worker on the go, keep reading this article. We’ll explore everything from meal prep to creating an ergonomic workspace to advocating for reasonable accommodations,

Specifically, this article touches on how thyroid disease can affect your work performance and offers some practical strategies to manage symptoms and promote productivity at work.

Thyroid disease has been reported to affect patients in terms of employment. According to a study published in the Journal of Clinical Endocrinology & Metabolism (JCEM), hyperthyroid people are more likely to take sick leave for extended periods than their healthy colleagues. This is because symptoms associated with thyroid disorders can affect an individual’s performance in the workplace, especially if the condition goes untreated.

Here are some common challenges individuals with thyroid disorders may face:

  • Fatigue: Fatigue is a common symptom of thyroid disorders, making it challenging to maintain energy levels throughout the workday. As a result, it can be hard to focus, meet deadlines, or participate actively in meetings.
  • Brain Fog: Difficulty concentrating and remembering details can affect your productivity and problem-solving skills.
  • Mood Swings: Fluctuations in thyroid hormone levels can contribute to mood swings, anxiety, and depression. These can affect your relationships with colleagues and your overall work satisfaction.
  • Temperature sensitivity: Feeling constantly cold or hot can be distracting and uncomfortable in a work environment.
  • Physical symptoms: Other physical symptoms such as muscle weakness, joint pain, and digestive issues (such as constipation and diarrhea) can also affect productivity and comfort in the workplace.

Strategies for Managing Thyroid Health for Productivity in the Workplace

Now that we understand how thyroid disease can affect work performance, it only makes sense to want to know if or how an individual can still be successful and productive at their job in spite of their thyroid condition. Actually, this is possible and there are several strategies individuals can employ to manage their health and thrive in the workplace

These strategies, discussed below, are categorized under four main pillars: communication, self-care, optimizing the work environment, and support systems.

1. Open Communication

  • Talk to your doctor: Keep in mind that apart from adherence to the prescribed treatment plan, consistent monitoring of thyroid function through blood tests is also important for managing your thyroid condition. For this reason, make it a point to always attend your scheduled appointments with your doctor. Discuss with your doctor the impact of your thyroid condition on your job. Your doctor may recommend some changes in your diet or medication dosage, or some other lifestyle modifications that can help you manage your condition better and improve your work performance.  

 

  • Inform your manager: If your thyroid condition affects your work performance, consider having an open and honest conversation with your employer or human resources department. It’s worth exploring any accommodations or adjustments that may help you perform your job more effectively, such as flexible work hours, telecommuting options (if your job allows), or ergonomic workspace modifications. In the United States, for instance, individuals with thyroid problems may qualify for protections and accommodations under the Americans with Disabilities Act (ADA) if their condition substantially limits one or more major life activities. Since disability laws vary worldwide and there is therefore no universal standard, it is best to research the specific laws in your country to know what your rights are exactly.

2. Self-Care

  • Prioritize your treatment: If prescribed medication for your thyroid condition, adhere to that treatment plan diligently. Sometimes, depending on your work hours or the general nature of your work, this may be especially challenging. For instance, a person who works more than one shift in a day or does not have a consistent daily work schedule is likely to have a constantly changing sleep schedule. If this person has hypothyroidism and has been prescribed levothyroxine, they are then tasked with having to aim for a consistent time window in relation to their last meal, regardless of the time of day. In this case, it might help to keep a supply of medication at work or set reminders to ensure consistency in medication intake. In any case, you should discuss your specific situation with your healthcare provider to find a strategy that works for you.

 

  • Adopt healthy eating habits: Healthy eating habits are good, not only for your thyroid health, but for your overall health too. Regardless of your thyroid condition, busy work schedules, especially for those on night shifts, can make consistent, healthy eating difficult. One way to work around this problem is meal prep. By devoting a few hours of one of your days off (weekends for most people) to prepare meals for the work week, you get to save time and get to control exactly what goes into your meals, ensuring they are rich in nutrients that support thyroid function, like selenium, zinc, and iodine. For safe consumption, thoroughly reheat leftover food until it’s steaming hot. This high temperature kills harmful bacteria that may have grown during storage. However, be careful to avoid reheating your food for longer periods than necessary as this can destroy the nutrients. Another benefit of meal prepping is that it allows you to portion out your food in advance, helping you avoid unhealthy overeating or skipping meals entirely. You can also pack healthy snacks like fruits, nuts, or yogurt to avoid unhealthy vending machine choices during long shifts.

 

  • Stay hydrated: Dehydration can exacerbate fatigue and impair cognitive function. Not only is dehydration capable of causing brain tissue to shrink, but just a 2% dip in hydration can impair your ability to concentrate, react quickly, and remember things. Drinking plenty of water can help with focus and energy, regardless of your specific thyroid condition. One useful tip is to keep a water bottle at your desk and hydrate regularly throughout the day. In case you’re wondering if you can still take some coffee even though caffeine intake has been associated with dehydration due to its diuretic effect, the answer is simple: as long as your daily caffeine intake isn’t excessive, you should be fine. In fact, in moderation, most healthy adults can safely enjoy up to 400mg of caffeine daily.

 

  • Maintain a healthy work-life balance: Establishing clear boundaries between work and personal life lies at the heart of an efficient and healthy work-life balance. Here’s how you can do this—disconnect from work emails and calls outside of work hours, and avoid checking work messages while on vacation. Also, don’t be scared to say no to additional responsibilities if they overload your schedule. If possible and when necessary, consider delegating some tasks that are likely to lead to a burnout. Moreover, research shows that engaging in hobbies outside of work, especially outdoor activities, can also be helpful by providing an outlet for stress relief, among many other health benefits. A study published in BMC Medicine in 2011 investigated the link between leisure activities and job satisfaction in doctors in the UK. They found that doctors who engage in more hobbies and leisure activities tend to report a greater sense of purpose and fulfillment in their work.

 

  • Exercise regularly: Regular physical activity can improve your energy levels, mood, and sleep quality. Aim for at least 30 minutes of moderate-intensity exercise most days of the week. However, at work, something as minimal as a brisk walk during lunch breaks can even help. Perhaps, rather than drive to go get lunch, you can try walking instead.

 

  • Monitor your symptoms: Keeping a journal to track your symptoms and how they affect your work is one useful habit you should adopt, if you haven’t already. Did a particularly busy day leave you feeling drained? Jot it down. Notice a difference in energy after starting a new medication? Jot it down.
